What are the ADA grab bar placement requirements?
According to the 2010 ADA Standards for Accessible Design:

Mounting height: 33″–36″ (840–915 mm) above the finished floor.

Extension requirements: - 
At least 12″ (305 mm) from the centerline of the toilet toward the side wall.
